How does fluid flow in the eye?
The eye constantly makes and drains a clear fluid called aqueous humor. The ciliary body behind the iris produces it, it flows forward through the pupil, and it exits through the trabecular meshwork — the mesh-like drain in the angle where the iris meets the cornea. Eye pressure is the balance between production and drainage. When the drain slows or blocks, pressure rises, and sustained high pressure damages the optic nerve. That is the link between fluid flow and glaucoma — and why most treatments work by improving drainage or reducing production.

To understand how glaucoma develops, it helps to know how fluid circulates within the eye. Fluid is produced inside of the eye, just beneath the iris, by a structure known as the ciliary body. The fluid then travels through the pupil, and exits via the eye's drainage system, called the Trabecular meshwork. In healthy eyes, there is a normal balance between the fluid that is made in the eye and the fluid that leaves the eye. Therefore, this fluid creates a relatively constant and healthy pressure within the eye. This is called intraocular pressure, or IOP, and it maintains the eye’s shape and function while the fluid nourishes the eye. Sometimes too much fluid is created, or the fluid does not drain fast enough, which can lead to an increase in eye pressure. Increased intraocular pressure can damage the optic nerve and lead to vision loss. This is why your doctor monitors your eye pressure, and why it is important to have routine eye exams.
